角膜塑型術治療近視壹年追蹤報告

Purpose : The safety and effectiveness of orthokeratology on the myopia reduction has been confirmed by various researchers. The objective of this investigation is to determine whether the orthokeratology effect on myopic reduction can be maintained long term.
Method : We investigated 21 myopic patients (35 eyes) ranged in age from 6 to 22 years, myopic refractive error ranged from –1.00 to –11.50 D and UCVA ranged from 20/50 to 20/2000. All subjects wore well-fitted ortho-k contact lenses (orthofocus B, Metro Optic, USA) and were monitored periodically for 1 years. At each follow up visits, the change of refractive error and UCVA were measured and recorded. ANOVA was used to examine these changes over time.
Results : Myopia was significantly reduced over 3 month study period (P<0.001, ANOVA) and followed a linear trend over time (P<0.005, trend analysis). The average reduction was 385.79 diopters with a range from –1.00 to –9.00 diopters. A total of 14(37%)eyes achieved a visual acuity of 20/20 or better and 20 (46%) eyes achieved 20/40 or better. If the patients stopped lens wear due to non-lens related problem, the myopia didn't revert to the pre-treatment level. When the lens wear resumed, the myopia reduced to the previous ortho-k treated level.
Conclusion : Myopia progresses most rapidly during adolescent stage if leave it untreated. The findings of this investigation suggest that the orthokeratology may be applied as a very effective alternative method to control and prevent myopia progression throughout the adolescent stage. Lens centration, comfort, visual quality and corneal integrity should be maintained at all times of lens wear. |
